Jury Duty Exemption Forms

If you believe you should be excused jury duty for health reasons you need to complete the Jury Duty Excusal form and provide this to us along with your jury duty citation letter from the court. You can do this by handing the documents in at the reception desk or by uploading the form and letter to us. The GP will provide a letter for you to give to the court if they agree that you should be excused jury duty on health grounds.

Your form should be ready for uplift with 15 working days (this maybe longer during periods of annual leave or illness).
