Insurance Reports

Completion of forms, certificates and medicals are not covered under the NHS, and the practice charges for carrying out this type of work. Please read the form carefully and fill in and sign any parts that you need to complete before you bring the form to the surgery. One of our Patient Services Advisers will be able to let you know what the cost is likely to be, and what you need to do to submit the request to us.

Please note that completion of forms, certificates, reports etc… can take up to 20 working days or more  for the process to be completed.
